Do you see the output in this same window? If so, does it state the version number as 1.2?
If you do get the window, and you are at v1.2, then it might be CocoaDialog which is missing. To test this, try running: type -p CocoaDialog«ctrl R» in an empty buffer.
CocoaDialog seems to be there.

Okay, that's the old Run Ruby which you're using.
If you're using latest build, go to ~/Library/Application Support/ TextMate/Bundles/Ruby.tmbundle, show pacakage contents and delete Commands/Run Ruby (/usr/local/bin).plist to reset to defaults.
