A folder is a mirror of what you have on disk. So e.g. if you drag the Documents folder (from your user directory) to the project drawer, it will show all the contents, and if you later change it, the contents will update in TextMate (or if you change it from within TextMate, it will change on disk).

So a folder in the project drawer is also always a folder on your harddisk.

A group on the other hand only exists in TextMate. If you drag individual files to TextMate's project drawer you can select to group them, name the group as you like, and the group only exists in TextMate, and the contents of the group won't change unless you change it.
