[TxMt] Markdown, UTF8 and RTF

Yes, insert ‘Format: complete’ in the top of your MultiMarkdown  
document.

This is required to make MultiMarkdown output the proper HTML  
boilerplate with encoding info.

Ideally the various conversion commands would check if headers are  
missing, and add it if the case.
