[SVN] ⌘R Related Shortcut Changes

Was brought up on IRC the other day that it might be best to move to  
using ⌘R in prose languages rather than the current web preview  
shortcut (⌃⌥⌘P). I went through and detailed the items that would  
be changed by this. Does this sound like a good change to everyone,  
any other issues?

## Move to ⌘R

* Preview (Markdown, Textile, Bulletin Board, MoinMoin, Txt2tags,  
Blogging, reStructuredText)
* Show {Upcoming Events, This Week, This Month} (Remind)
* Preview (CSS) Note: Unsure what to do here in the case of embedded  
CSS.
* Preview (HTML) Note: New command, would need to be another shortcut  
until PHP is split off.
* Run (PHP)


## Move to ⌘⇧R

* Generate Output and Open in Browser (Markdown → MultiMarkdown)
* Convert to HTML and Preview in Browser (Txt2tags)
* Preview in Browser (reStructuredText)


## Move to ⌃⇧H

* Convert to Default Target (Txt2tags)
* Create ---- Document (reStructuredText)


### Conflicts

* Reload Document (MoinMoin)
* Convert to Default Target (Txt2Tags)
* Create ---- Document (reStructuredText)
* Refresh Running Browser(s) (source.css scope)
* Refresh Running Browser(s) (HTML) Note: Move to ⌘⇧R ?
