@Ed:
Actually the key combo CTRL+ESC can't be used in TM for a user-defined  
command.
And AFAIK /Applications/TextMate.app/Contents/SharedSupport/Support/ 
lib/dialog.rb is obsolete.

Which TM version do you are using?

Did you try Bundles > Select Bundle Item... ?
