This is what I get when I start TM:
2007-02-05 21:10:51.637 TextMate[626] init load themes

same bundle identifier (com.macromates.dialog_plug-in) already loaded

Don't really have any useful ideas (the rebooting suggestion was kind  
of a last resort ;), I didn't think it would work either).Perhaps  
someone more knowledgeable with the workings of the plugin can offer  
some more ideas.

Let's compare md5 values and make sure we are talking about the same  
file at least:
