I don't know why, but I never tried Smooth Scrolling before.
After reading your post I tried it with a few apps and it work in
almost all Text views/Lists views. (It's not working in Camino and
FF.)

Then I tried in TM... and it works too.
It's working really well with page Up/Down, I don't see any difference
using the scroll wheel though, the difference is more obvious in e.g.
Safari or TextEdit.

Might just be a pref to set for document windows...
