General cleanup of the scopes so other bundle items can better target their scopes.

• Change trailing scope to .tm-grammar (Making the scopeName source.plist.tm-grammar)
• Change meta.rule.repository to meta.dictionary.repository
• Add meta.array.patterns scope to the pattern arrays.
• Add meta.dictionary.rule for items within a pattern array.
• Change meta.rule.captures to meta.dictionary.captures
• Change meta.rule to meta.value-pair
• Change meta.rule.repository-item to meta.value-pair.repository-item
