Rewrite the 'insert comment header' snippet as a language-neutral command.
• Support all languages
• Support both line- and begin/end-style comments
• Use dscl to find username, niutil is deprecated
• File and project names are snippetized if they couldn't be filled in automagically
• Fall back to username for copyright assignment if there is no TM_ORGANIZATION_NAME
