Merge in needed changes to match recent updates to various scopes.

• Change string.regexp constant.character.escaped to string.regexp constant.character.escape
• Change other.preprocessor.c to meta.preprocessor.c
• Change other.preprocessor.c entity to meta.preprocessor.c entity
• Change declaration.* to meta.*
• Change entity.other.namespace to entity.name.tag.namespace
• Remove outdated marup.hr rule.
• Change meta.preprocessor.c entity to meta.preprocessor.c keyword
• Change entity.name.tag.namespace to entity.name.tag.namespace, entity.other.attribute-name.namespace
