Patch from Bob Frank ≺bob_frank ⊛ mac ⊚ com≻:
• do not increase indent for the document environment (\begin{document} … \end{document})
• do not treat % as a comment starter when preceded by a backslash (\)

A remaining issue:
• an additional newline should not be introduced between a commented line and a following (originally non-empty) line, since this will yield an additional paragraph break for (La)TeX.
